Exercises for Hiatal Hernia
How these exercises can help
These exercises focus on diaphragmatic breathing, rib-cage mobility, abdominal pressure control and posture after meals. With regular practice, they may improve breathing mechanics and trunk coordination and help reduce reflux-related discomfort.
Where to start
Choose the symptom that is most noticeable today.
Diaphragm · RefluxDiaphragmatic breathing
PositionLie on your back with your knees bent and avoid doing the exercise straight after eating. If lying down worsens reflux, sit with your back supported instead.
How to do itPlace one hand on your chest and the other on your upper abdomen. Breathe in through the nose for about 4 seconds, then breathe out slowly for about 6. Let the abdomen and lower ribs move with the breath.
Why it may helpThis encourages diaphragmatic movement and may support the diaphragm’s contribution to the anti-reflux barrier. It can also reduce unnecessary tension during breathing.
When5 minutes once or twice a day. Stop if it causes burning, dizziness or breathlessness.5-minute breathing practice
Breathe in for 4 seconds and out for 6. Keep the breathing relaxed and do not force the timing.

Neck · BreathingPosterior neck stretch
PositionSit upright and let your chin drop slightly towards your chest. Rest your hands behind your head without pulling.
How to do itHold for 20 to 30 seconds. As you breathe out, relax the muscles at the back of the neck. Let your hands rest there without pulling.
Why it may helpThis can reduce tension in the back of the neck and limit overuse of the accessory breathing muscles.
WhenBefore the breathing exercise if your neck or shoulders feel tense.
Breathing controlPursed-lip breathing
PositionSit or stand upright with your shoulders relaxed.
How to do itBreathe in through the nose, then breathe out slowly through pursed lips. Let the air flow out without forcing it.
Why it may helpLengthening the out-breath can slow the breathing rate and reduce unnecessary effort in the neck and shoulders.
When6 to 10 breaths whenever you want to slow the breathing rhythm.
Thorax · PostureSeated thoracic extension
PositionSit near the edge of a chair with your hands behind your head and your elbows open.
How to do itBreathe in as you lift the breastbone and extend the upper back. Breathe out as you return to the starting position. Keep the movement in the upper back and avoid arching through the lower back.
Why it may helpImproving rib-cage mobility gives the diaphragm more room to move during breathing.
When6 to 8 repetitions, especially after sitting for a long time.
Coordination · PressureLower abdominal activation on exhalation
PositionLie on your back with your knees bent, away from meals.
How to do itAs you breathe out, draw in the lower abdomen slightly. Do not brace, strain or hold your breath. Relax fully as you breathe in.
Why it may helpThis coordinates the abdominal wall with the out-breath without straining or holding the breath.
When5 to 8 repetitions. Stop if you notice increased pressure or discomfort.
Movement · Post-mealWalking after meals
PositionStay upright and avoid tight clothing or belts that press on the abdomen.
How to do itWalk for 10 to 15 minutes at your usual pace. There is no need to walk fast.
Why it may helpA short walk keeps you upright and avoids the compression that can make reflux worse after eating.
WhenAfter meals, provided walking does not increase symptoms. Avoid lying down straight after eating.
When to stop
Stop and get medical advice if you develop difficulty swallowing, gastrointestinal bleeding, unexplained weight loss, persistent vomiting, new chest pain or symptoms that are clearly getting worse.
